The maximum inherent damage mod linked to health is +15% damage when health is greater than 50%, or +20% damage when health is less than 50%.

A 1x% always mod will come with a corresponding negative as well. Either -5 energy, -10 armor while attacking, -1 energy degen, -1 health degen. While there a few, very rare, always damage items, they are extremely expensive.
